Privacy Policy

A straightforward explanation of the information FormDino handles, why we use it, and the choices available to account holders and form submitters.

Scope and who we are

This Privacy Policy explains how Ann Arbor Apps LLC, doing business as FormDino (“FormDino,” “we,” “us,” or “our”), handles personal information when people visit our websites, create or use a FormDino account, build or publish a form, submit a form, or interact with our related services.

FormDino is an evolving product. This policy applies to new features and services that link to it unless we provide a different notice for a particular feature.

Information we collect

  • Account information. Name, email address, profile image, account identifiers, sign-in provider, organization details, and account preferences.
  • Forms and Tracks. Form questions, settings, embed information, and the submissions recorded for a form. The FormDino customer decides what a form asks and what a submitter provides.
  • Billing information. Plan, subscription, invoice, transaction, and payment status, plus the status and identifiers needed to connect a form owner’s Stripe account and match a visitor payment to a Track. Payment card details are handled by Stripe; FormDino does not store full card numbers.
  • Usage and device information. Pages and features used, approximate location derived from IP address, browser, device, timestamps, referring pages, cookie or similar identifiers, and diagnostic information.
  • Embedded-form activity. Approximate form loads aggregated by Egg, day, referring website hostname, customer-supplied Property label, and installation method. These view totals are not designed to identify a visitor or represent unique people.
  • Communications. Messages, support requests, survey responses, and other information you choose to send us.

We receive information directly from you, from people who submit forms, automatically when the service is used, and from services you choose to connect, including identity and payment providers.

How we use information

  • Provide, authenticate, maintain, and improve FormDino.
  • Save, publish, display, and process forms and their Tracks.
  • Manage plans, trials, billing, limits, accounts, and support.
  • Understand product usage and develop new features.
  • Communicate about the service, transactions, and policy changes.
  • Detect and address fraud, misuse, technical problems, and legal obligations.

Form owners and submitters

A FormDino customer controls the forms they publish and decides why submission information is collected and how it is used. For that information, FormDino generally acts as a service provider or processor on the customer’s behalf.

If you submitted a form and want to access, correct, or delete that submission, contact the person or organization that published the form first. We will assist FormDino customers with valid requests as required by applicable law.

When an Egg requires payment, the form owner is the merchant and Stripe processes the payment on that owner’s connected account. FormDino stores limited payment status and reference information so the owner can match the payment to the applicable Track; FormDino does not receive or store the visitor’s full card number.

How we share information

We may share information with:

  • Form owners and authorized members who can access the applicable account, workspace, form, or Track.
  • Service providers that support hosting, cloud infrastructure, content delivery, analytics, authentication, payments, communications, and customer support.
  • Authorities or other parties when reasonably necessary to comply with law, protect rights, investigate misuse, or respond to an emergency.
  • A successor or transaction participant in connection with a financing, reorganization, sale, merger, or transfer of all or part of our business, subject to appropriate safeguards.
  • Other parties when you direct us or give consent.

We do not currently sell personal information for money. If our practices change in a way that creates new legal rights, we will update this notice and provide any controls required by law.

Cookies and analytics

FormDino and its providers may use cookies, local storage, and similar technologies to keep sessions working, remember settings, measure use, and understand how the service performs. Our public website currently uses Google Analytics.

When a FormDino form or enhanced installation loads, it may send a limited activity signal so the form owner can see approximate view totals and notice unusual volume. FormDino aggregates that signal without storing a raw IP address, user-agent string, cookie, or persistent visitor identifier in the form-view record.

Browser settings and available product controls may let you limit these technologies. Blocking essential storage may prevent sign-in or other service functions from working correctly.

Retention and protection

We keep information for as long as reasonably needed to provide the service, maintain business and transaction records, comply with law, resolve disputes, and enforce agreements. Retention depends on the kind of information, why it was collected, account settings, and legal requirements. Copies may remain temporarily in backups and logs after deletion.

We use reasonable administrative, technical, and organizational measures designed to protect information. No online service can guarantee absolute security, and customers are responsible for protecting their credentials and configuring account access appropriately.

Your choices and rights

Depending on where you live, you may have rights to request access, correction, deletion, portability, restriction, or objection regarding personal information, and to appeal a decision. We may need to verify your identity and may retain information where permitted or required by law.

Account holders can manage certain information in the Nest or contact us. Form submitters should contact the form owner first. You can also disconnect an identity provider through that provider’s account controls.

International use

FormDino and its providers may process information in the United States and other countries where privacy laws may differ. Where required, we use recognized transfer mechanisms or other appropriate safeguards.

Children

FormDino is not directed to children under 13, and they may not create an account. Customers may not use FormDino to collect children’s personal information unless they have all authority, notices, consents, and safeguards required by law. Contact us if you believe information was collected in violation of this section.

Changes to this policy

We may update this policy as FormDino, our providers, or applicable laws change. We will post the revised policy with a new effective date and, when required, provide additional notice through the service or by email.
